Essential neoplasms of the sphenoid sinus are uncommon. Metastatic injuries are experienced even less oftentimes. We present an instance of metastasis of pancreatic neuroendocrine carcinoma to the sphenoid sinus in a 18 year old male and to report this uncommon yet conceivably befuddling radiologic finding.
